Registered Nurse – Geriatric Nurse (FT)

Position: Registered Nurse – Geriatric Nurse

Department: Human Services

Status: Full Time - RN

Hours: 36 hours per week, Hours may be adjusted in response to workload demands. Low census may be utilized in accordance with hospital policy.

Qualifications/Position Requirements:

  1. Graduate of an accredited School of Nursing
  2. Current Michigan Board of Nursing license as a Registered Nurse with required renewals
  3. Knowledge of current nursing standards of practice
  4. BLS Certification required within 3 months of hire and recertification as required
  5. Ability to establish effective working relationships and effective communication with peers, patients, families, physicians, and all other support hospital & external personnel
  6. Ability to function in a leadership position by serving as a resource person, assuming leadership role in emergency/crisis situation and delegating specific tasks to volunteers who might help.
  7. Ability to influence the direction of professional nursing practice by maintaining a positive perspective/attitude and participation in the promotion of change to keep current with health care
  8. Organized and has the ability to manage multiple tasks in a stressful situation while maintaining composure and excellence in the delivery of McKenzie’s Senior Services.
  9. Supports and contributes to the total hospital wide system by attending and supporting various county wide senior citizen groups such as Building Ties, including attending the Sanilac County Senior Fair, and participating in the Alzheimer Support Group.
  10. Ability to provide and promote excellence in the delivery of services to both internal and external customers
  11. Desire to be part of a health care team dedicated to providing the community and patients optimal care and excellent service
  12. A sense of responsibility, initiative, honesty, and a positive attitude
  13. Proficiency utilizing software programs and has the ability to work in an Electronic Medical Record.

Job Summary:

A Registered Nurse (RN) who is responsible for McKenzie’s Senior Services Program, as funded by Sanilac County Council on Aging through: 1. Completing home visits for the aging population within Sanilac County. Maintaining a chart on each patient and recording each visit as well as notes on phone calls, etc. with patient, family, and physicians. Addressing in home safety, compliance, wellness, and direct care services for older adults on a long-term basis. Completes assessment and monitors patient for changes via screening tools such as, but not limited to, the mini mental status exam, Tinetti Balance test, and geriatric depression screening. Complete medication review and reconciliation with provider. Educate on disease process and monitor for acute episodes of disease process. Provide family conferencing and resources to patient and family. Identifying and addressing social isolation and in-home safety checks. Encouraging food and fluid intake while identifying and addressing barriers to food access. Identifying and reporting notable changes to primary caregiver and/or patient’s provider; 2. Attends monthly Council on Aging meeting to report on the services that are being offered to seniors in Sanilac County. Attend meal sites (Deckerville, Sandusky, Marlette, and Lexington) monthly to offer vital checks, monitoring of any changes, and offer education to participants. 3. Complete instructor training and host exercise classes such as, but not limited to, Matter of Balance, Tai Chi, and Enhanced Fitness. While also completing all documentation and reporting requirements for funding as required by Region 7 Area Agency on Aging.
