Requesting Health Records:
The Health Information Department will be happy to assist you in obtaining your health information. Health records must be requested by the person they belong to or their legal representative according to HIPAA Federal Laws. There may be a fee for copies of health records. Please allow seven business days for requests to be completed.
The following items are needed in order for your health records to be released:

Signed Authorization for Release of Information
- This document must be filled out completely and signed by the patient, or the patient’s legal representative.
- Copy of drivers license or photo ID
- If you are sending someone on your behalf to pick up your records, you must provide the person’s name on the Authorization for Release of Information form. The designated person must bring photo identification in order for records to be released.
- If you are a legal representative of a patient, please bring documentation attesting to your authority.
